Shopian, Oct 09: Following the death of a 27-year-old woman at the Community Health Centre (CHC) Keller, the Chief Medical Officer (CMO) Shopian has constituted a three-member committee to investigate the circumstances leading to her death and determine the exact cause.
According to the order issued by the CMO Shopian, the deceased has been identified as Shazia Waseem, wife of Waseem Ahmad Lone, a resident of Chawan Keller. She was admitted to CHC Keller on October 8, 2025, and reportedly passed away during the night between October 8 and 9.
The enquiry committee will be chaired by Block Medical Officer (BMO) Shopian, Dr. M. Yousuf Naikoo, and includes Dr. Samiullah Dar (Consultant Gynecologist, CHC Zainapora), Dr. Seth Mujtaba (Surgeon Specialist, District Hospital Shopian), and Dr. Shabir Ahmad Langoo (Consultant Anesthesia, CHC Zainapora).

As per the official order, the committee has been tasked with examining all medical records, treatment protocols, and case sheets related to the deceased patient. It will ascertain the sequence of events, evaluate the treatment provided, and determine whether standard medical guidelines and procedures were followed at CHC Keller.
The committee will also identify any possible negligence, lapse, or systemic failure on part of the hospital staff and recommend corrective actions, including administrative or disciplinary measures if required.
The CMO has directed the committee to start the enquiry immediately and submit a detailed report within three days. The Block Medical Officer Keller has been instructed to provide all necessary records and logistical support to facilitate the investigation. (KNT)
